intake form allows you to start the application process by entering your preferences and needs so that we (agents) can help you find the doctors, medicines, hospitals, that are important for you. Keep in mind that some Insurance plans may not cover all your preferences.. that's why its reccomended you do the intake form first so we can help you find the plans that will best match your needs
